Homeownership Program Application Instructions
To be considered for Habitat for Humanity’s Homeownership Program, this online application must be fully completed and submitted along with all required supporting documents.
After we receive your completed application and all documentation, you will be contacted within 4–6 weeks with an update on your eligibility and next steps.
A background check and credit check will be required to proceed with the application process.

If you prefer, you may download a printable version of the entire application, fill it out by hand, and submit it along with all required documents in person at our office.
Please note: There are two versions of the application—one for individuals applying on their own and one for those applying with a co-applicant. If you are married, you must apply with your spouse as your co-applicant.

Please note: You must upload scanned or electronic copies only. No photos will be accepted.
The following documents are required for each application:
Photo ID for all household members age 18+
(Color copy of driver’s license, state-issued ID, or, if not a U.S. citizen, front and back of permanent resident card)Birth Certificates for everyone who will be living in the home
Proof of Income for all household members
If employed:
Most recent 90 days of pay stubs
Two most recent tax returns, including W-2s
If self-employed:
Two most recent tax returns, including 1099s and Schedule C
Current year-to-date profit and loss statement
If receiving benefits (Social Security, Disability, Retirement):
Current award letter showing monthly amount and duration, if applicable
Alimony/Child Support:
Court order showing amount awarded
Case history showing amounts disbursed
If not court-ordered, provide 6 months of payment history
If divorced, submit divorce decree
Other assistance (if applicable):
Award letters for HUD, SNAP, food stamps, housing, etc.
Proof of mortgage denial
A letter from a bank or mortgage company showing you do not qualify for a conventional or government-assisted mortgage.Bank Statements
Most recent 6 months of statements for all checking, savings, and other bank accounts for all household members401(k) Statements, if applicable
Please review the qualification criteria before beginning the application to ensure you meet the basic eligibility requirements.
Disclaimer- In some instances, minimum income levels may not qualify for certain areas/markets or high valued/priced homes.
